Corrective actions should always address the 3-legged 5-why in 2 areas (Process issues aka why built. Detection aka why not caught in inspection or shipped... And Systemic issues aka why not well-defined, clear responsibility).

I have dealt with several methodologies of corrective actions in the past several years and the method that made the most sense (which was much easier to understand for more people in our organization) was the methodology introduced by former Denso employee.

2007年10月10日 · I particularly like the 3 legged 5 Why analysis tool. If you haven't heard of it, the first leg is 5 why's of the actual problem or issue first. The second focuses on the controls - lack of em actually - escape routes if you will and the third focuses on the systemic causes.
