This guide is designed to aid primary care physicians investigating hyperferritinaemia, the main causes of which are listed in Table 1. overleaf. The ferritin range in the Central Manchester University Hospital laboratory are 30 ‐ 400 μg/l for a male and 13 ‐ 150 μg/l for a female.

Under normal conditions the levels of ferritin reflect the amount of storage iron that is present in the body. Low levels are consistent with iron deficiency. Elevated levels are seen when body iron stores are increased.

High serum ferritin is found in a large spectrum of conditions which may or may not represent iron overload. Usually ferritin levels will be above 1000 ug/l in clinically established haemochromatosis. Serum ferritin is now the test of choice for diagnosing iron deficiency anaemia. Please note, ferritin is an acute phase protein and may be affected by intercurrent illness.

Less than 30 microgram/L has a 92% sensitivity and 98% specificity for diagnosing iron deficiency and a ferritin less than 40 microgram/L is 98% sensitivity and specific for iron deficiency. A ferritin above 50 microgram/L excludes iron deficiency unless there is inflammation.

Serum ferritin concentrations are high at birth, but fall during the first few months and remain low in childhood. In healthy adults the level varies with age and gender.
