In 1986, Bigliani and colleagues [3] were the first to classify acromial morphology and correlate it with rotator cuff tears. They described three types of acromion: Type I (flat), Type II (curved), and Type III (hooked) (Fig. 1). A Type IV acromion was later described as a convex acromion [28].

Bigliani et al 5 classified the shape of the acromion on supraspinatus outlet (or lateral) radiographs into three distinct types: type I represents a flat, type II a curved and type III a hooked undersurface of the acromion (Fig. 1a).

In the majority of people the undersurface of the acromion is flat (type 1), but for some people the undersurface can be curved (type 2), or hooked (type 3), with type 2 and 3 acromions reducing the space available for the tendons and the bursa (see Figure 2 below).

Type I acromion is characterized by a flat and smooth shape, with a relatively straight or slightly curved undersurface. It has minimal or no protrusion and is considered the most desirable and least associated with shoulder impingement or rotator cuff problems.
