Case study 138 - Far Lateral Disc Herniation Far lateral disc herniations account for 0.7%–12% of lumbar disc herniations and most commonly occur at the L4–5 and L3–4 levels. Far lateral and extreme lateral disc herniations are defined by extrusion of disc material lateral to the intervertebral foramen that migrate superolaterally from the disc space of origin. This can result in compression of the adjacent exiting nerve root from the level above. They can be difficult to deal with surgically. #
